Section 2.0: Clinical practice points

<http://hollandbloorview.ca/Assets/website/documents/Teaching%20and%20learning%20documents/Evidence%20to%20Care/Section2.0.pdf> – using recommendations from the Registered Nurses’ Association of Ontario’s best practice guideline for the Assessment and Management of Pain (3rd edition), clinical experts have devised practice points to guide evidence-informed decision making when assessing chronic pain in children with communicative, cognitive and physical impairments.

Section 3.0: Pediatric chronic pain assessment tools <http://hollandbloorview.ca/Assets/website/documents/Teaching%20and%20learning%20documents/Evidence%20to%20Care/Section3.0_RENEW.pdf> – outlines the 15 pediatric chronic pain assessment tools that were selected for inclusion in the Toolbox, 7 of which examine chronic pain interference and 8 address chronic pain coping.

Section 4.0: Implementation supports <http://hollandbloorview.ca/Assets/website/documents/Teaching%20and%20learning%20documents/Evidence%20to%20Care/Section4.0.pdf> – based upon the Knowledge to Action cycle (Graham et al., 2006), this manual provides a step-by-step guide for the adoption, implementation and evaluation of the Toolbox at your organization.
