Nominations are now being accepted for projects in four
areas of care: Staying Health, Getting Better, Living with Illness, Coping
with End of Life.
Nominations can also be made for individuals who have
demonstrated Leadership in Advancing the
Patient Voice, Leadership in Quality,
for an Everyday Champion or a Quality Culture Trailblazer.

Winners
receive a $2,500 sponsorship to support or disseminate learning from their projects,
and will be recognized with an awards ceremony at Health Talks on March 1,
2017.
Winners will also receive complimentary registration for the Quality Forum on March 2 & 3, 2017, and winners of the four initiative-based awards will be invited to present at the Forum.
