• Section 230 should include:
• Type of injury including details. E.g. Use “right below knee amputation” vs. “amputation”
• Area of concern - e.g. “ABI, with concerns regarding diplopia, attention & problem-solving, further recovery is expected”
• Date of injury or date of diagnosis
• Concurrent medical history. If substance abuse, OSA, diabetes or other diagnoses are present , indicate if these are treated and stable
